I confirm that I have not been told by my Doctor that I have a heart condition. I have not felt pain in my chest when I exercise or in the last three months. I do not know of any reason why I should not do exercise. I am aware that if I have any physical symptoms during or after the training session it is my responsibility to mention it to the trainer. I am aware of my own limits and will work within these.

One Element Group Limited will use personal data about you either for the purpose of providing park-based fitness (including free trials) and social and sporting events as well as, for our internal administration and management purposes, and analysis (to better understand our customers and their requirements). We will treat all of this information in confidence and keep it secure. We may need to share it with our staff, franchisees (including One Element Ltd), trainers, MailChimp, Team Up, GoCardless for the purpose of helping us to provide our services. Membership Agreement
By registering as a member of One Element, or any of the One Element franchises which trade as One Element, you are agreeing to accept the terms of this Membership Agreement. We strongly recommend that you print out and save a copy of your Membership Agreement and contact us in case of any issues. We will keep a record of your acceptance of this Membership Agreement. The medical waiver form that you complete on joining as a Member shall form part of this Membership Agreement.
